/upload_session/finish incorrect_offset

Hello, I'm trying to upload with /upload_session/append_v2 and /upload_session/finish Sometimes it works and sometimes I got incorrect_offset error.   I cannot understand why. Can you tell me more...

    I had an error on append_v2 that's why the offset was not correct.
