On my new iPhone 13 Mini with iOS 15 (15.0, 15.0.1 and 15.0.2 all have this issue) the background uploads don't work reliably. New photos may get occasionally uploaded in the background with a long delay, but usually the new photos will be uploaded only when I open the Dropbox app manually.
Under Camera Uploads the settings are as follows:
- Back up: "All photos"
- Upload from: "Camera Roll"
- Include videos: ON
- Use cell data to back up: ON
- Save HEIC photos as: JPG
With the same settings photos and videos were uploaded nicely in the background on my previous iPhone XS at least until iOS 14.something, never updated it to iOS 15 though.
Any ideas how to get the background uploads to work again?

Have you tried disabling your camera uploads, then enabling it?
Also, I'd try clearing your app's cache, to see if this changes how it behaves. Want to give it a go?

I tried both of the suggestions, turned it off and back on again as well as cleared the cache, but no change in behavior. I have to open the app before it starts uploading.
I've got "always" selected there now - I believe this was set originally to "never", then I changed it to "allow while using app" and as it didn't help, I changed it to "always" but still no luck whatsoever. I believe my old phone also had this setting as "always". Here's a screenshot of this view (in Finnish though):
