    Hi there, we’re assuming you’re referring to the iOS Mail app, not the Mac OSX app.
     
    Have you installed the Dropbox mobile app, and does the Dropbox folder appear on the Files app on your iPad?
     
    If not, could you confirm what version of iOS you’re running on the iPad, and what version of the Dropbox app is installed?

I've noticed the same thing, where the files that will attach have the download symbol.  But, this is also inconsistent.  Sometimes it will, sometimes it won't.

If the Files app isn’t updating with the latest version of the file as it appears in the Dropbox mobile app, then this would be related to the Files app itself.
 
There isn’t much we can troubleshoot here since the integration is handled through that app. 
 
You can try deleting the Dropbox mobile app, rebooting the device, and reinstalling the app to see if this helps however.
