Data is missing off the signed document

I am creating lease agreements - that have a start date for the lease, which is manually enterered into the document before its uploaded into dropbox. The signed copy has no date

Is this happening for a single document, or for most documents that have been signed?

Were the signees able to see the date, while signing the document? 

Besides the date, are there any other fields that seem to be missing?
